In order to fly a glider, you must have a certain level of medical fitness, and you need to say what that status is. There are three options:
A. I declare that I satisfy the medical requirements displayed on the medical Declaration of Physical Fitness form (see below), and am fit to fly a glider
B. I will be assessed by a GP against the Medical Practitioner’s Certificate of Fitness for the issue of a private motor vehicle driver’s licence; (see below) or
C. I Hold a valid CASA Medical Certificate

If you are under 18, you MUST download the paper declaration form and have it signed by parent/guardian, and email it to email@example.com.
If you are self-declaring your status a form is not required unless you are under 18yo (see below).
If you choose Option B or C then you MUST provide the GFA with a copy of your medical documents - this can be done either by uploading the scanned documents using the File Upload button below, or by emailing the documents to firstname.lastname@example.org.
One of options B or C is necessary if you are an Instructor, a Charter pilot, or suffer from certain medical conditions listed on the medical Self Declaration of Physical fitness form. Please note that you MUST provide the GFA with a copy of your medical documents - this can be done either by uploading the scanned documents using the File Upload button in this renewal form, or by emailing the documents to email@example.com.
